Please also bring in or attach any relevant results from pathology / blood testing by your doctor.

Should you be unable to attend your appointment for any reason, please telephone at least 24 hours in advance as there is usually a waiting list. A 50% fee will be charged for missed appointments with no notification. For your convenience we have eftpos and credit card facilities available.
